外阴鳞状细胞癌的预后因素分析

Prognostic factors of vulvar squamous cell carcinoma

目的:通过Cox模型探讨影响外阴鳞癌预后的主要因素。方法:选取67例经手术治疗的外阴鳞癌患者,选择年龄、职业、生活所在地、外阴瘙痒时间、肿物增长时间、外阴营养不良、产次数、大体类型、肿瘤大小、肿瘤部位、手术种类、预后分数、术后放疗、存活或死亡以及存活时间(年)等变量,用Cox模型进行单因素及多因素分析。结果:单因素分析结果显示:预后评分高、术后未接受放疗,位于阴蒂头和外阴瘙痒时间长的患者预后较差(P<0.05)。多因素分析结果显示,预后评分高,外阴瘙痒时间长,位于大阴唇或阴蒂头部位肿瘤和产次数增加是影响外阴癌预后的最重要的危险因素(P<0.05)。结论:外阴鳞状细胞癌的临床分期、病理分级、淋巴结转移、肿瘤部位、外阴瘙痒时间和产次数是影响预后的主要因素。

Objective：To investigate the main factors of vulvar squamous cell carcinoma prognosis by analysing the Cox model.Methods：To select 67 patients with vulvar squamous cell carcinoma after surgery.The variables such as age,occupation,living area,duration of vulvar itching,time of tumor growing,vulvar dystrophy,times of labor,general type,size and location of tumor,type of surgery,prognostic score,radiotherapy after surgery,duration of survival and death were input into the IBM-PC computer.To analyze the single and multiple factors by the Cox model.Results：The single factor analysis：high prognosis scores,no radiotherapy after surgery,long duration of itching of clitoris and labium majus had poor prognosis;the multivariate analysis：high prognostic scores,long duration of itching of the vulva,labium majus and clitoris were the most significant risk factors affecting prognosis（P〈0.05）.Conclusion：The clinical stage,pathological type,lymphatic metastasis,site of tumor,duration of itching of the vulva,labium majus and clitoris,times of labor are the main factors affecting prognosis.
