青年患者与中老年患者胃癌临床对比分析

Comparative study on clinical characteristics of gastric carcinoma between young and elder patients

目的分析青年患者胃癌的临床特点。方法回顾性分析我院23例40岁以下胃癌患者的临床资料并与同期325例40岁以上胃癌患者进行了对比分析。结果本组女性多于男性(1.30∶1);肿瘤好发于胃窦部;均为进展期,其中BorrmannⅡ、Ⅲ型(30.43%,56.52%)为主;以低分化腺癌及印戒细胞癌多见(分别占43.48%和39.13%)。全组患者均行手术,其中根治性切除12例(52.17%),明显低于老年组(71.08%,P<0.05);青年组根治术后1、3、5年总体生存率分别为82.41%、52.35%、27.60%,其中3、5年生存率低于中老年组(86.42%、60.13%、41.20%),差异有统计学意义。结论青年患者胃癌发病率女性多于男性;病理类型以低分化腺癌和印戒细胞癌为多;根治性切除的机会低于中老年人;根治性切除术后的3、5年的生存率低于中老年人。

Objective To investigate the clinical characterstics in young gastric carcinoma cases. Methods Clinical date of patients with gastric carcinoma （23 cases aged below 40 years old and 325 cases aged above 40 years old）,were analyzed retrospectively and contrastively. Results The ratio of male to female for all cases was 1∶1.30. Most of the tumors located in gastric antrum, and all cases were at advanced stage according to clinical pathology, BorrmannⅡ, Ⅲ （accounting for 30.43% and 56.52%, respectively）type occupied the dominant percentage and poorly differentiated adenocarcinoma and ring cell carcinoma were the dominant pathological type （accounting for 43.48% and 39.13% , respectively）. All cases were treated with operation, 12 cases in young group were treated with the radical resection （52.17%）, which was significantly different with that in old group （71.08%）. The 1, 3, 5-year survival rate of radical resection cases in young group were 82.41%, 52.35%, 27.60% and in old group were 86.42%,60.13%,41.20%, and 3, 5-year survival rate were different between the two groups （P〈0.05）. Conclusion The female young gastric carcinoma cases is more than male young gastric carcinoma cases, and the poorly differentiated adenocarcinoma and ring cell carcinoma are the dominant pathological type. The ratio of radical resection in young is lower than that in elderly and after radical resection, 3-year and 5-year survival rate of the young cases are lower than that of the elderly cases.
