非透视下闭合穿“V”形钉治疗胫腓骨骨折

CLOSE REDUCTION AND INTERNAL FIXATION WITH V SHAPED INTRAMEDULLARY NAIL FOR THE TREATMENT OF FRACTURE OF TIBIA AND FIBULA WITHOUT THE HELP OF X RAY

目的：报告非透视下闭合穿“Ｖ”形钉治疗胫腓骨骨折；方法：“Ｖ”形钉两端适当向前折弯，经胫骨结节上方斜坡入路，保持“Ｖ”形钉与胫骨嵴一致，整复骨折后在非透视下将钉打入。１９９４～１９９８年，用此法治疗胫腓骨骨折３５例，上１／３，８例，中１／３，１４例，下１／３，１３例；结果：３４例闭合穿钉成功，１例由于肢体肿胀和软组织嵌入骨折片间而切开复位。随访～５年，骨折均在３个月内愈合，无断钉、弯钉情况，无膝痛症状；结论：非透视下闭合穿“Ｖ”形钉治疗胫腓骨骨折，操作易，创伤小，疗效好，避免了Ｘ线对人体的损伤，对手术室无“Ｃ”臂Ｘ光机设备的医院更为实用。

Objective:To report the treatment of fracture of tibia and fibula by close reduction and internal fixation with V shaped nail without the help of X ray.Method:Through the oblique surface above tbial tubercle ,the nail which two ends were bent forward properly was kept in line with tibial crest and then was struck into marrow cavity of tibia carefully after reduction without the help of X ray.35 cases of fracture of tibia and fibula were treated with this method from 1994 to 1998.Among them the fracture of the upper one third of tibia was in 8 cases,the middle one third 14 cases ,the lower one third 13 cases.Result:34 cases were successful in close reduction and intrernal fixation .One case had to be given an open reduction due to severe crural swelling and intercalation of soft tissue between the fractured fragmemts.The follow up were of 1 to 5 years.The fractures were all healed within 3 months without breaking or bending of the nail or any pain in the knee joint.Conclusion:The close V shaped nailing for the treatment of fracture of tibia and fibula without the help of X ray is convenient,less traumatic and excellently effective.The human body damage brought about by X rays is avoided.It is more practical to the hospital in which there is no X ray instrument in the operation room.
