顺行股前外侧岛状皮瓣和肌皮瓣的临床应用

Clinical application of retrograde anteriolateral femoral island cutaneous flaps and myocutaneous flaps

目的探讨应用顺行股前外侧岛状皮瓣和肌皮瓣修复髂部、大转子部和腹股沟等处软组织缺损的临床效果。方法应用顺行股前外侧岛状皮瓣和肌皮瓣修复１２例软组织缺损，其中，岛状皮瓣９例，肌皮瓣３例。供区直接缝合。结果除１例岛状皮瓣远端发生尖部坏死经植皮愈合，其余病例术后全部成活。随访１～５年，平均２５年，供区愈合良好，受区外形、质地及厚薄均较满意。结论该皮瓣以旋股外侧动脉降支为其血供，具有血运丰富、血管解剖恒定、血管蒂长和切取较易等优点，适宜修复大转子和髂部等处软组织缺损。

bjective To study the clinical results of repairing soft tissue defect of ilium, greater trochanter and groin by retrograde anteriolateral femoral island cutaneous flaps and myocutaneous flaps. Methods 12 cases soft tissue defect were repaired by the technique. Among them, island cutaneous flaps were 9 cases and myocutaneous flaps 3 cases. The size of island flaps ranged from 16 cm9 cm to 9 cm7 cm.The donor sites were closed directly. Results 1 flap developed distal tip necrosis,which was managed by skin graft and the rest survived with satisfactory results. Conclusion The flap is nourished by desending branch of lateral circumflex femoral artery.The island flaps seem to have good vascularization, a constant vascular anastomy and a long vascular pedicle so that flap dissection could be accomplished easily. The flap is suitable for repair of soft tissue defect of the greater trochanter and ilium.
