摘要
目的探讨湿疹患儿肠道菌群改变及其与T辅助细胞Th1/Th2平衡的相关性。方法选取湿疹患儿22例。观察其肠道菌群主要组成成分双歧杆菌、乳酸杆菌、大肠杆菌和肠球菌的数量,并与性别、年龄相匹配的健康对照组儿童比较。对湿疹患儿肠道内双歧杆菌和大肠杆菌数量对数值的比值(B/E值)与外周血单核细胞分泌的Th1细胞因子IFN-γ、Th2细胞因子IL-4水平行相关性分析。结果 1.与健康对照组比较,湿疹患儿粪便中双歧杆菌、乳酸杆菌数量显著下降(Pa=0.000),大肠杆菌数量明显上升(P=0.000),肠球菌数量变化无统计学差异(P=0.163)。湿疹患儿B/E值为0.77±0.60,显著低于健康对照组(1.34±0.26)(P=0.000)。2.相关分析表明湿疹患儿中肠道菌群B/E值与外周血单核细胞分泌的IL-4水平呈显著负相关(r=-0.603,P=0.008),但与IFN-γ水平无明显相关性(r=0.212,P=0.150)。结论湿疹患儿体内存在明显的菌群紊乱,肠道菌群紊乱可能影响机体Th1/Th2细胞功能平衡。
Objective To explore the changes of intestinal flora in children with eczema and the correlation between intestinal flora and Th1/Th2 balance.Methods The feces from the children with eczema and from the healthy children matched in age and sex were collected,a quantitative analysis on bifidobacterium,lactobacillus,colibacillus and enterococcus were made,and the correlation between intestinal flora and the levels of immunoreactive fibronection-γ(IFN-γ)and interleukin-4(IL-4)secreted by peripheral blood mononuclear cells were analyzed.Results 1.Compared with the healthy control group,children with eczema possessed significantly lower counts of bifidobacteria and lactobacillus,and significantly higher counts of colibacillus in their feces(Pa=0.000),but there was no significant difference in enterococcus between the 2 groups,the intestinal flora B/E in patients(0.77±0.60)was significantly lower than that in the control group(1.34±0.26)(P=0.000).2.In eczema patients,there was significantly negative correlation between IL-4 levels and B/E(r=-0.603,P=0.008),and no correlation was found between IFN-γ and B/E(r=0.212,P=0.150).Conclusion There is a disturbed intestinal flora in children with eczema and this disturbing can affect the balance of Th1/Th2.
