糖尿病患者颌面颈部间隙感染的临床研究

A clinical analysis of maxillofacial and cervical space infections with diabetes

目的:探讨糖尿病患者颌面颈部间隙感染的临床特点及治疗要点。方法:对2004-06～2009-06我科住院的36例伴有糖尿病的颌面颈部间隙感染患者的临床资料进行回顾性分析。结果:36例患者均行手术切开引流,其中8例行气管切开,经围手术期监测调整血糖,全程足量使用抗生素,所有患者未出现严重的全身或局部并发症,均痊愈出院。结论:对于糖尿病患者的颌面颈部间隙感染,治疗上控制血糖和控制感染并重,经验用药的选择需考虑优势菌肺炎克雷伯菌,对于牙源性感染,需加用抗厌氧菌药物。

Objective：To investigate the clinical feature and treatment of maxillofacial and cervical space infections with diabetes.Methods：Clinical data of 36 cases from June 2004 to June 2009 were retrospectively analyzed.Results：Surgical drainage was performed in all patients and tracheotomy was performed in 8 patients.All patients were recovered without severe systemic and local complications by controlling blood sugar and applying sufficient antibiotics during perioperative period.Conclusion：In the treatment of maxillofacial and cervical space infections with diabetes,control of blood sugar is as important as control of infection.The choice of empirical antibiotics agents should be K.pneumonia.In patients with an obvious odontogenic source of infection,empirical antibiotics covering anaerobic infections are necessary.
