阑尾恶性肿瘤16例临床诊疗分析

Clinical analysis of 16 patients with primary appendix tumor

目的:分析阑尾恶性肿瘤的临床特征,治疗方法及预后。方法:回顾分析1999年至2009年间16例罕见的阑尾恶性肿瘤的临床资料。结果:16例病人发病年龄23-86岁,中位年龄51岁,男:女为5:11。所有患者均行手术治疗。阑尾类癌预后较好,黏液性腺癌预后次之,腺癌预后差。结论:原发性阑尾肿瘤发病率低,术前诊断困难。对于有浸润倾向的恶性肿瘤应行右半结肠切除术,对于黏液性腺癌,应避免手术中黏液外漏。腹腔化疗和全身化疗可以改善晚期阑尾肿瘤病人的生存质量。

Objective：To sumarize the clinical features of primary appendix tumor and it＇s key points of diagnosis and treatment.Methods：Total of 16 patients with primary appendix tumor treated in our hospital from 1999 to 2009 were analyzed.Results：The incidence of primary carcinoma of appendix is very low.The media age at the time of presentation was 51（23-86）,male vs femal was 5 vs 11.Operation was performed in all patients.The prognosis of appendix carcinoid was better than appendix mucinous adenocarcinoma,the prognosis of appendix adenocarcinoma was worst.Conclusion：Primary carcinoma of appendix is uncommon and difficult to be dignosed exactly.Right hemicolectomy should be performed for appendix carcinoma,which has a tendency of metastasis.Mucus should not be lekaged in section for appendix mucinous adenocarcinoma.Intraperitoneal chemotherapy and systemic chemotherapy for advanced appendix carcinoma patients can improve their life qualities.
