1995～2009年临沂市乙肝疫苗免疫接种及其效果分析

Analysis on Immunization Effect of Hepatitis B Vaccine in Linyi City,1995-2009

[目的]了解和评价乙肝疫苗预防接种效果,为更好地制定实施乙肝免疫预防策略。[方法]对1995～2009年临沂市新生儿乙肝疫苗接种和3～18岁人群血清HBsAg检测资料进行分析。[结果]新生儿乙肝疫苗报告接种率,1995年为89.45%,2000年为82.53%,其他年份为92.07%～99.96%;首针及时接种率,2000～2009年分别为82.53%、89.85%、90.08%、99.19%、99.89%、98.48%、93.05%、98.37%、98.45%、98.43%。3～18岁儿童HBsAg阳性率,1995年为7.93%,2004年为3.61%,2009年为2.27%(P<0.01);3次调查中均有随着年龄的增长而增高的趋势(P<0.01)。3～14岁儿童抗-HBs阳性率,1995年为23.64%,2009年为69.60%(P<0.01)。[结论]临沂市新生儿乙肝疫苗接种率已经达到99%以上,首针及时接种率达到98%以上;3～18岁人群HBsAg阳性率逐渐降低,抗-HBs阳性率逐渐升高。

[Objective] To know and estimate prevention effectiveness of hepatitis B vaccine in order to formulate and implement vaccination strategies preferably for hepatitis B.[Methods]Analysis was made on data of hepatitis B vaccination among neonates and HBsAg serological surveillance data of 3-18 age groups in Linyi city from 1995 to 2009.[Results]the coverage rate of neonates ＇ hepatitis B vaccine was 89.45% in 1995,82.53% in 2000 and 92.07%-99.96% in other years.The timely first dose rate was 82.53%,89.85%,90.08%,99.19%,99.89%,98.48%,93.05%,98.37%,98.45%,and 98.43% from 2000 to 2009 respectively.The positive rate of HBsAg among 3-18 was 7.93% in 1995,3.61% in 2004,2.27% in 2009（P0.01）and decreased with the increase of age year by year（P0.01）.The positive rate of HBsAb among 3-14 was 23.64% in 1995,69.60% in 2000（P0.01）.[Conclusion]The coverage rate of hepatitis B Vaccine has reached 99% and the timely first dose rate increases to 98% in Linyi city.The positive rate of HBsAg among 3-18 has decreased and the positive rate of HBsAb rises gradually.
