摘要
目的 观察免疫肠内营养(IEN)在全胃切除术后患者中的应用效果,并与标准肠内营养(SEN)和肠外营养(PN)作比较,评价术后IEN应用的优势.方法 将87例全胃切除术后患者随机分为PN组、SEN组和IEN组,每组29例.比较3组患者术后第1、7、10天营养、免疫等指标的恢复情况.结果 IEN组术后第7、10天在提高术后营养、纠正负氮平衡和各项免疫指标等方面较SEN组及PN组有效,差异均有统计学意义(均P〈0.05),且IEN组术后感染性并发症发生率低于SEN组及PN组(P〈0.05).结论 全胃切除患者术后应用IEN对蛋白质合成及免疫功能恢复疗效显著,并能降低术后感染性并发症发生率.
Objective To evaluate the application of postoperative enteral immunonutrition for patients with total gastrec- tomy. Methods Eighty-seven patients with total gastrectomy were randomly divided into 3 groups: immune-enhancing enteral nutrition group (IEN group, n=27), standard enteral nutrition group (SEN group, n=27) and parenteral nutrition group (PN group, n=27). Nutrient status and immune function of 3 groups were examined on the post-operative d 1, 7 and 10. Results The postoperative nutrient status, immune function and nitrogen balance in IEN group were more markedly improved at postoperative d7 and dl0 than those in SEN and PN groups (P〈0.05). The rate of postoperative infective complications in IEN group was lower than that in SEN and PN group (P〈0.05). Conclusion Postoperative enteral immunonutrition is safe and effective for patients with total gastrectomy.
