斯普林联合化疗治疗老年晚期胃癌

Sipulin injection combined with chemotherapy in elderly advanced gastric cancer

目的探讨斯普林注射液辅助化疗治疗老年晚期胃癌的近期疗效及其对免疫功能的影响。方法 83例老年晚期胃癌分为两组,斯普林联合化疗组（联合组）42例,采用FOLFOX 4方案治疗,化疗第1天斯普林10m l静脉滴注,d1-d10;对照组41例,单用FOLFOX 4方案化疗。14天为1周期,4周期后分别对疗效、免疫功能以及不良反应进行评价。结果联合组总有效率为45.2%,高于对照组的41.4%,但差异无统计学意义（P〉0.05）;疾病控制率斯普林联合化疗组为73.8%,优于单纯化疗组的58.5%,差异有统计学意义（P〈0.05）;联合组和对照组中位疾病进展时间分别为8.5个月和7.8个月,差异无统计学意义（P〉0.05）;联合组治疗后CD3、CD4阳性细胞百分率以及CD4/CD8比值明显提高,与治疗前以及对照组治疗后比较,差异均有统计学意义（P〈0.05）。结论斯普林注射液联合化疗治疗老年晚期胃癌的疗效略有提高,不良反应减轻,机体免疫力得到明显改善。

Objective To explore the short-term efficacy and its impact on immune function for sipulin injection combined with chemotherapy in elderly patients with advanced gastric cancer.Methods Eighty-three elderly patients with advanced gastric cancer were divided into two groups,in which 42 cases using sipulin combined with FOLFOX 4 regimen and 41 cases using FOLFOX 4 regimen only.Sipulin 10ml was infused intravenously from day 1 to day 10.The control group of 41 patients achieved chemotherapy alone.Fourteen days was a cycle,the short-term effect of the treatment,peripheral blood leukocytes,hemoglobin,platelet,immune function were evaluated after 4 cycles.Results The total effective rate of sipulin combined with chemotherapy（45.2%） was higher than chemotherapy alone group（41.4%）,but wasn＇t statistically significant difference（P〉0.05）,median time to progression were 8.5 months and 7.8 months months,respectively,without statistically significant difference（P〈0.05）;while the clinical benefit rate of sipulin,the combination group（73.8%） was superior to chemotherapy alone group（58.5%）,which was significant difference（P〈0.05）;indicators of T-cell immune subsets： CD3,CD4 positive cells percentage,CD4/CD8 ratio significantly increased,the difference was significant between the two groups（P〈0.05）.Conclusion Sipulin injection combined with chemotherapy treatment of elderly advanced gastric cancer can increase efficacy,reduce adverse reactions,improve immunity and chemotherapy tolerance.
