摘要
目的探讨逆向双"Z"形瓣成形术在腭裂修复术中的的临床应用价值。方法应用逆向双"Z"形瓣成形术,于软腭口腔面和鼻腔面各作一方向相反、层次不一的"Z"形黏膜肌瓣交叉移位,以达到肌纤维复位和延长软腭的目的。临床共治疗21例,男性12例,女性9例,年龄16～24个月,平均18.6个月;其中不完全性腭裂13例,单侧完全性腭裂8例。结果 21例患者切口均一期愈合,术后3～12个月对21例患者进行回访,无复裂及腭瘘形成,术后语音功能明显改善。其中13例行鼻咽纤维内镜观察,显示腭咽闭合良好。结论腭裂手术中加用逆向双"Z"形瓣成形术,可达到良好的腭咽闭合效果,符合功能性腭裂整复术的原则要求,是较理想的腭裂修复方法。
Objective To explore the clinical application value of double opposing Z-plasty in repairing surgery of cleft palate.Methods We have repaired 21 patients using double opposing Z-plasty from June 2008 to July 2010,including 12males and 9females,aged 14 months to24 months（mean 16.8months）.Results All patients have healed perfectly,there is none case occurred postoperative perforation.Speech sound and velopharyngealfunction have been impoved with different degrees,after 3 to 12 months fllowing up.13 patients received the nasopharyngo scopy,of which velopharyngeal closure was corrected completely.Conclusion It is feasible for repairing the cleft palate through double opposing Z-plasty which meet the requirements of the functional restoration of cleft palate
