摘要
目的防止大鼠原位肝移植术后肝上下腔静脉吻合口撕裂出血,减少受体血管重建后吻合口远侧已形成血栓引起的肝坏死、下肢瘫痪和肺栓塞等严重并发症。方法参照Kamada介绍的方法,着重对肝上下腔静脉、门静脉和肝下下腔静脉吻合方法作了进一步改进。结果供肝热缺血时间2.5~3.5分钟,平均3.0分钟,安装袖套管时间约2.0分钟,肝上下腔静脉吻合时间平均为8.5分钟,供肝冷缺血时间23.0~30.5分钟,无肝期9.0~12.0分钟,1周存活率88%。结论改进的血管吻合方法提高了手术成功率。
Objective To prevent SHVC anastomotic site from laceration and decrease severe
complications such as liver necrosis,hind limb paralysis and pulmonary embolism caused by
thrombosis preformed at anastomotic stoma after recipient's vascular reconstruction. Methods
Based on Kamada's method,we modified the anastomoses of suprahepatic vena cava,portal
vein and infrahepaticvena cava. ResultsThe anhepatic period was 912 min,and the warm and
cold ischemic time was 2.53.5 min and 23.030.5 min respectively.88% of the grafted animals
survived for 1 week.Conclusion This led to an increased operation successful rate in orthotopic
liver transplantation in the rat.
