摘要
目的探讨胰腺损伤的诊断方法和治疗原则。方法回顾性分析1989—2009年收治的39例胰腺损伤患者的临床资料,总结胰腺损伤的诊断和处理方法,并对术后并发症进行分析。结果 39例胰腺损伤患者中,开放性损伤18例,闭合性损伤21例。8例闭合性损伤患者术前经B超明确诊断,18例行CT检查16例发现胰间质水肿或胰头体或尾挫伤或断裂伤。结论对于开放性胰腺损伤,有明显腹部症状的闭合性胰腺损伤,疑有Ⅰb、Ⅰc、Ⅱ型胰腺损伤均应及时行剖腹探查,根据胰腺损伤的程度选择合理的术式,减少术后并发症的发生。
Objective Toinvestigets the principle of the diagnosis and treatment for pancreatics trauma.Methods Retrospective analgsis onethe 39 patients who suffened form pancreatic trauma between1989 and 2009.Results In 39 patients with pancreatic injury,open injury in 18 cases,21 cases of blunt trauma.Eight cases of blunt trauma patients,preoperative diagnosis by B-18 found in a routine CT examination of 16 cases of pancreatic interstitial edema,or body or tail pancreatic contusion or rupture.Conclusion The indications for exploratory operation.Should be considered when open pancreatis trauma closed pancreatis trauma Comhined with obvious symptoms type.Ⅰb Ⅰc Ⅱ Suspected by CT examination were determired the rational selecting operative prodedures for the extent of the pancreatis trauma to posto perative complioation.
