双层螺旋CT增强扫描在闭合性肝损伤诊断中的价值

Double spiral CT enhanced scanning for the diagnosis of blunt hepatic injury

目的评价双层螺旋CT增强扫描对闭合性肝脏损伤的诊断价值及临床意义。方法回顾性分析2006年9月至2009年8月间收治的75例急性闭合性肝脏损伤的CT征象,患者临床资料完整。结果 75例CT平扫发现肝脏明确损伤61例(81.33%),显示肝包膜破口9例(12.00%),肝脏可疑损伤14例(18.67%)。增强扫描:门静脉期清晰显示肝包膜破口17例,14例可疑损伤均明确显示为相对低密度挫伤灶,显示率为100%。结论双层螺旋CT增强扫描门静脉期能够更准确地判断肝脏损伤的存在并予以CT分级,特别是对肝包膜破裂及肝实质内小的不明显损伤灶的显示更具有价值。

Objective To evaluate the double spiral CT enhanced scanning in the diagnosis of blunt hepatic injury and its clinical significance.Methods Retrospective analysis of CT signs of 75 blunt hepatic injury cases with whole clinical data from September 2006 to Augest 2009.Results In 75 cases scanned by CT, 61 cases were found clear hepatic injury（81.33%）,9 cases with liver capsule rupture（12.00%）,and 14 cases with suspicious damage of liver（18.67%）.Enhanced Scanning：portal venous phase clearly indicated that 17 cases with liver capsule rupture,all the 14 cases with suspicious injuries indicated relatively low-density contusion lesion.Conclusions Double-helical CT scans portal venous phase enhanced to more accurately determine the presence of liver injury and be CT grade;in particular,rupture of the liver capsule and hepatic parenchymal injury in the small kitchen obvious display of more valuable.
