下斜肌前转位治疗伴有下斜肌功能亢进的垂直分离性斜视的疗效观察

The effect observation of anterior tran-sposition of inferior oblique muscle treated in dissociated vertical deviation association with overaction of inferior oblique muscle

目的分析下斜肌前转位(anterior tran-sposition of inferior oblique muscle)治疗伴有下斜肌功能亢进的垂直分离性斜视(Dissociated vertical deviation,DVD)的疗效。方法伴用下斜肌前转位治疗伴有下斜肌功能亢进的DVD患者9例(12只眼)。术后随访0.5～6个月,评价斜视度数和眼肌功能。结果术前DVD平均19.7PD,术后平均3.6PD。术前下斜肌功能亢进3+和2+各6只眼,术后仍有2只眼(16.7%,2/12)下斜肌功能亢进,但为1+。无任何患者主诉原在位时有复视或垂直斜视,术后1只眼(8.3%,1/12)有轻度上转受限。结论下斜肌前转位治疗伴有下斜肌功能亢进的DVD有效。

Objective To evaluate the efficacy of anterior transposition of the inferior oblique muscle for treatment of eyes with dissociated vertical deviation（DVD） association with overaction of inferior oblique muscle.Methods Surgery was performed on 12 eyes of 9 patients with DVD associated with overaction of inferior oblique muscle.The surgical results were evalvated at 0.5 to 6 months after surgery,evaluation of strabismus degree and muscle function.Results Preoperatively,the mean DVD measured 17.9 PD.It had to 3.6 PD after the operation.Before the operation,6 eyes had 3 ＋ and 6 eyes had 2 ＋ inferior oblique muscle overaction.After the operation,only 2 eyes（16.7% ） had 1 ＋ inferior oblique muscle overaction.None of the patients developed dioplopia or hypotropia in the primary position,after operation,1 eye（8.3% ） had mild limitation on the transfer.Conclusion Anterior tran-sposition of the inferior oblique muscle is an effective treatment for DVD associated with overaction of inferior oblique muscle.
