慢性乙型重型肝炎辨证分型研究

A study on the dialectical typing of chronic severe hepatitis B

目的:初步探讨慢性乙型重型肝炎的辨证分型规律,以指导中医药对慢性乙型重型肝炎的治疗。方法:对300例慢性乙型重型肝炎患者中医四诊信息进行统计学处理,对整理后的证候变量进行样品系统聚类分析,并结合中医理论及专家辨证,确定各聚类类别。结果:慢性乙型重型肝炎中医证型可聚为七类:寒湿困脾、湿热内蕴、热入心包、瘀热互结、肝肾阴虚、脾肾阳虚、寒热夹杂;总体证型分布情况:湿热内蕴>寒湿困脾>热入心包>脾肾阳虚>瘀热互结>寒热夹杂>肝肾阴虚。结论:初步揭示了慢性乙型重型肝炎的辨证分型规律,为慢性乙型重型肝炎辨证分型标准的最终确立并广泛应用于临床提供依据。

Objective： This study aims to investigate the dialectical typing rules of chronic severe hepatitis B so as to offer some guidance for Chinese medicine to treat the chronic severe hepatitis B.Methods： By making a definite diagnosis of each cluster category through the sample analysis with hierarchical Clustering Methods on the reduced data of symptom variables combined with some traditional Chinese medicine theories and dialectic results from some experts,which is based on the descriptive statistical analysis of the four diagnostic information from 300 patients with chronic severe hepatitis B.Results： Chinese patients with chronic type B hepatitis can be divided into seven categories： colddamp invasion of spleen,the interior wet and heat,heat attacking pericardium,Mixed Stasis-Heat,liver-kidney yin deficiency,spleen-kidney yang deficiency,combination of Cold and Heat;overall disease type Distribution：the interior wet and heatcold-damp invasion of spleenheat attack-ing pericardiumspleen-kidney yang deficiency Mixed Stasis-Heatcombination of Cold and Heatliver-kidney yin deficiency.Conclusion： It initially reveals the dialectic typing laws of chronic hepatitis type B,which provids some basis for us to establish the standard to definite the dialectic typing of chronic severe hepatitis B and also offers evidence for us to make widely use of it in clinical diagnosis eventually.
