消化系统恶性肿瘤患者化疗期抑郁与自我效能应对方式社会支持相关性分析

Correlation among Depression and Self-efficacy, Coping Style and Social Support on Patients with Digestive System Malignant Tumor during Chemotherapy

目的探讨消化系统恶性肿瘤患者化疗期抑郁的发生情况及相关因素。方法采用Zung氏抑郁自评量表、一般自我效能量表、简易应对方式问卷量表、社会支持评定量表及自设问卷,评价66例化疗期的消化系统恶性肿瘤患者抑郁发生情况,并分析其相关性。结果 37例(56%)患者出现抑郁症状,医疗负担很大、化疗第1和第2疗程、化疗效果恶化、不良反应重的患者抑郁发生比例高(P<0.05)。抑郁与消极应对得分呈正相关(P<0.05),与一般自我效能得分、社会支持评定总分、客观支持得分、主观支持得分、对支持的利用度和积极应对得分均呈负相关(P<0.05)。结论消化系统恶性肿瘤患者化疗期抑郁发生率高,尤其应关注医疗负担很大、化疗第1及第2疗程、化疗效果恶化、不良反应重的患者。此外,应对方式消极、自我效能水平低、社会支持缺乏的患者更容易发生抑郁。

Objective To explore depression and related factors on patients with digestive tract malignant tumor during chemotherapy.Methods 66 patients with digestive tract malignant tumor during chemotherapy were surveyed with Zung’s Self-Rating Depression Scale（SDS）,General Self-Efficacy Scale（GSES）,Simple Coping Style Questionnaire（SCSQ）,Social Support Rating Scale（SSRS） and self-develop questionnaire to estimate depression and its correlation factors.Results Thirty-seven patients（56%） were with depression.Patients with heavy medical burden,in the first and second period of chemotherapy and patients with aggravated and having severe complications after chemotherapy were with a high level of depression（P 0.05）.The degree of depression was positively related with the negative coping score（P0.05）,negatively related with the score of GSES,the total score of the SSRS,subjective support score,objective support score,the support exploitability and the positive coping score of SCSQ（P0.05）.Conclusion Patients with digestive tract malignant tumor during chemotherapy have a higher occurrence rate of depression,particularly for patients with heavy medical burden,in the first and second period of chemotherapy and patients with aggravated and having severe complications after chemotherapy.Furthermore,patients with negative coping style,lower self-efficacy and short of social support have more potential to be depressed.
