^(125)I粒子植入术治疗门静脉癌栓40例近期疗效观察

Short-term Efficacy of ^(125)I Seed Implantation Treatment for Portal Vein Tumor Thrombus

目的:探讨CT导向下^(125)I粒子植入治疗门静脉癌栓的临床价值和操作流程。方法:选取40例原发性肝细胞癌伴门静脉癌栓患者,采用TPS计算布源,在CT导向下将^(125)I粒子植入瘤灶内,采用(2.2～3.3)×10~7 Mq活度的^(125)I粒子多层面播植。结果:40例患者中癌栓完全缓解18例(45.0%),部分缓解22例(55.0%),总有效率100%。所有患者均未发生手术相关严重并发症。结论:CT导向下^(125)I粒子植入治疗门静脉癌栓近期效果好,安全性高,创伤小,并发症发生率低。

Objective： To investigate the clinical value of CT-guided ^125Ⅰ radioactive seed implantation in treating portal vein tumor thrombus. Methods： A total of 40 hepatocellular carcinoma （HCC） patients with portal vein tumor thrombus were collected. Under CT guidance,^125Ⅰseeds of 2.2×10^7 to 3.3×10^7 Mq were implanted into the metastatic lymph nodes according to TPS. Results： Of the 40 patients, 18 cases （45.0%） achieved complete remission （CR）, and 22 cases （55.0%） achieved partial remission （PR）. The overall response rate was 100%. No serious complications were observed. Conclusion： CT-guided ^125Ⅰ radioactive seed implantation is effective and safe in treating portal vein tumor thrombus with minimal damage and few complications.
