血清瘦素水平与颅内动脉狭窄相关性研究

摘要 目的探讨血清瘦素水平与颅内动脉狭窄的相关性。方法选取60例颅内动脉狭窄患者及60例无颅内动脉狭窄者,分别测定其血清瘦素浓度(Leptin)、体重指数(body mass index,BMI)、收缩压(systolic blood pressure,SBp)、舒张压(diastolic blood pressure,DBp)、空腹血糖(fasting blood glucose,FBG)、甘油三脂(triglyeride,TG),总胆固醇(total cholesterol,TC),低密度脂蛋白(low-density lipoprotein,LDL),高密度脂蛋白(high density lipoprotein,HDL),空腹胰岛素(fasting insulinum,FIN)等指标,血清瘦素用酶联免疫吸附方法(enzyme linked immunosorbent assay,ELISA)测定,使用稳态模式评估法(homeostasis model assessment,HOMA)评价并计算胰岛素抵抗指数(HOMA-IR)=空腹血糖(FBG)×空腹胰岛素(FIN)/22.5。分析血清瘦素在实验组及对照组间的差异,及瘦素与其他生化指标的相关性。结果实验组LDL、TG、FIN、HOMA-IR、Leptin水平均明显高于对照组(P<0.05),其中Leptin水平差异尤为显著(P<0.01);血清瘦素水平与体重指数、空腹血糖、总胆固醇、胰岛素抵抗指数成正相关(r>0.4,P<0.01),和年龄、血压及空腹胰岛素成正相关;多因素Logistic回归分析表明年龄、BMI,FBG、HOMA-IR及Leptin与颅内动脉狭窄有相关性。结论颅内动脉狭窄与瘦素有一定的正相关性,高瘦素是颅内动脉狭窄的危险因素之一。 Objective To investigate the relationship between leptin and intracal arterial stenosis.Method 60 patients with intracranial arterial stenosis were included as the case group,while 60 subjects without intracranial arterial stenosis matched with the case group on gender and age were selected as control group.Respectively,leptin were measured by enzyme linked immunosorbent assay(ELISA).Level of BMI,SBp,DBp,FBG,TG,TC,FIN,LDL,HDL and HOMA-IR were measured as well.Results Plasma level of leptin in case group was significantly higher than that in control group(19.76±5.89 vs 14.18±3.60,P〈0.01),and plasma level of LDL,TG,FIN and HOMA-IR were higher than that in control group(P〈0.05).Significant positive correlation were observed between plasma level of leptin and plasma level of FBG,TC,as well as BMI and HOMA-IR.Based on logistic regression model,high plasma level of leptin was one of the risk factors.Conclusion There was the significant positive correlation between leptin and intracranial arterial stenosis.
