摘要
目的评价空气贮留、肺结核与肺功能指数的相关性。方法通过对53例病理或临床证实的肺结核患者,使用螺旋CT分别进行了不同呼吸时相的连续容积数据模式扫描,对肺结核和空气贮留分别进行评分,全部患者作了肺功能检查。结果肺结核和空气贮留分别在234/318和176/318个肺叶中见到,两者平均积分高度相关(r=0.74,P<0.01)。空气贮留积分与肺功能指数相关性:第1秒最大呼气量占用力肺活量百分比(FEV1/FVC)(r=-0.40,P<0.05)、25%肺活量时最大用力呼气流率(FEF25%)(r=-0.50,P<0.05)相关,第1秒最大呼气量(FEV1)(r=-0.47,P<0.01)、用力肺活量(FVC)(r=-0.45,P<0.01)、50%肺活量时最大呼气流率(FEF50%)(r=-0.76,P<0.01)、残气量(RV)(r=0.78,P<0.01)高度相关。结论在肺结核患者中空气贮留与小气道功能指数具有密切相关性。
Objective To investigate the correlations between air trapping as well as tuberculosis and lung fnction indexes. Methods 53 patients with tuberculosis diagnosed by clinical data and pathology were scanned by using spiral CT at end inspiration and expiration in continuous volume scan mode. The expiration scans were scored at a lobar level for the extent of tuberculosis and air trapping. All the patients were performed pulmonary function tests. Results Tuberculosis and air trapping were found in 234/318 and 176/318 pulmonary lobars. The mean air trapping scores were closely related to mean tuberculosis scores ( P 〈 O. O1 ). The correlations between lung fnction indexes and and air trapping score were FEV1/FVC ( P 〈 0.05) , FEF 25% ( P 〈 0.05 ) , FEVI( P 〈0. 01),FVC( P 〈0. 01),FEF50%( P 〈0. 01),RV( P 〈0. 01).Conelusion There is adefinite correlation between air trapping and tuberculosis small airways' function indexes in patients with tuberculosis.
