实施基本医疗卫生制度对卫生技术人员工作行为的影响

The Impact of Implementation of Basic Medical and Health Care System on the Behavior of Health Professionals in Community Health Organizations

目的:分析医疗卫生制度实施对基层卫生技术人员提供医疗和防保服务的影响。方法:实验研究前后,分别利用机构调查表,获取基层卫生机构医疗及防保工作的基本数据;利用处方分析法,分析卫生技术人员行医行为的变化,并对卫生技术人员进行定性访谈。结果:与对照县相比,项目县基层卫生技术人员用药量减少,基本药物利用率提高;抗生素使用率、多联抗生素使用率以及静脉输液率下降;公共卫生服务开展的范围和数量增加,卫生服务质量有所提高。结论:基本医疗卫生制度的实施,卫生技术人员的诊疗行为更加规范,更加重视公共卫生工作。

Objectives： To analyze the impact of implementation of basic medical and health care system on the behavior of health professionals in providing medical and public health services.Methods： Before and after the pilot study,community health organiza-tions were investigated using questionnaire in both experimental counties and control counties.Prescription analysis was also used to show the change of clinical behavior,and qualitative interview was also introduced in the survey.Results： Comparing with control counties,the quantity of drug usage and basic drugs per prescription decreased,the application of antibiotics and multifold antibiotics also reduced.The scope and quantity of public health services provision increased,and the quality of both medical services and public health services improved.Conclusions： The implementation of basic medical and health care system,standardized the clinic behavior of health professionals in community health organizations,and public health are paid more attention to.
