脑卒中后抑郁症52例临床分析

Clinical analysis and treatment of 52 cases of depression after stroke

目的:探讨脑卒中后抑郁的发病率及相关因素,并了解帕罗西汀治疗PSD的临床疗效。方法:将52例脑卒中后抑郁患者中随机分为治疗组和对照组,对照组予以常规神经系统药物和支持治疗。治疗组在与对照组相同治疗的基础上加用帕罗西汀20～40mg/d,疗程均为8周。运用通用测评方法,分别记录两组患者分别在治疗前及治疗8周后的各项指标,并对其进行统计分析。结果:本次研究抑郁发生率为36.88%,治疗8周后,两组患者的上述指标比较,差异均有统计学意义(P<0.05),两组患者的HAMD、NIHSS评分明显下降,且治疗后,治疗组HAMD、NIHSS评分低于对照组HAMD、NIHSS评分,差异有统计学意义(P<0.05),治疗组治疗后有效率为92.31%(24/26),对照组治疗后有效率为53.85%(14/26),且两组差异有统计学意义(P<0.05)。结论:帕罗西汀可有效改善脑卒中后抑郁症患者的抑郁情绪,并能改善患者神经功能及日常生活能力,且不良反应少,安全性较好。

Objective：To investigate the incidence of post-stroke depression and related factors,and understand the paroxetine clinical effect of PSD.Methods：52 patients with depression after stroke patients were randomly divided into treatment group and control group and the control group received conventional treatment of nervous system drugs and support.Treatment group and control group on the basis of the same treatment plus paroxetine 20 to 40 mg/d,treatment was 8 weeks.The use of common evaluation methods,two groups of patients were recorded before and after treatment,respectively,after 8 weeks of the index,and its statistical analysis.Results：The incidence of depression was 36.88%,after 8 weeks treatment,both groups of the above indicators,the difference was significant（P0.05）,two groups of patients with HAMD,NIHSS score was significantly decreased,and the treatment treated group HAMD,NIHSS score lower than the control group HAMD,NIHSS score,the difference was statistically significant（P 0.05）,after treatment,the effective rate was 92.31%（24/26）,the effective rate of the control group after treatment 53.85%（14/26）,and the difference was statistically significant（P0.05）.Conclusion：Paroxetine is effective in improving depression in patients with post-stroke depression,and can improve neurological function and activities of daily living,and adverse reactions,safety is good.
