关于涂阴肺结核病临床研究

目的涂阴肺结核病临床特征和诊断及疗程的研究。方法回顾分析3年期间,诊断涂阴肺结核676例,对其临床表现、X线特征等动态观察及治疗全程,第2、5、6个月末痰涂片检查。结果涂阴肺结核88.9%,临床症状以咳嗽、咯血为主,但症状稍微,另11.1%无任何症状,在体检时发现。常规剂量抗痨治疗全程后,疗程末症状改善,X线显示吸收好转率100%。结论涂阴肺结核临床症状稍微,以咳嗽、咯血、低热、盗汗为诊断线索,结合X线、实验室检查并排除其它疾病而综合判定。X线是诊断涂阴肺结核重要手段之一[1]。这部分涂阴病人给予6个月治疗方案,以6～9个月治疗方案最为多见。目前国内外有关材料表明：有相当数量,约50%～70%痰涂片阴性病例存在[2],在结核病控制工作中,这部分病人是否是传染源,是否需要治疗,是值得探究的新问题,回顾3年间收治涂阴肺结核676例,就其临床特征、诊断和治疗分析如下。

Objective Purpose of the clinical characteristics of smear negative TB,diagnosis and treatment research.Methods Retrospective analysis of three years period,676 cases of smear negative pulmonary tuberculosis diagnosis,the clinical,X-ray characteristics of the dynamic observation and treatment of the whole,the first in February,May,June at the end of sputum smear examination.Results 88.9% of smear negative pulmonary tuberculosis,clinical symptoms were cough,haemoptysis,but the symptoms slightly,and the other 11.1% without any symptoms,the examination found.Conventional dose of antituberculosis treatment in the whole,the course of last symptoms,X ray showed improved absorption rate of 100%.Conclusion The clinical symptoms of a slight smear negative pulmonary tuberculosis,cough,hemoptysis,fever,night sweats for diagnostic clues,combined with X ray,laboratory tests and the exclusion of other diseases and comprehensive determination.X line is an important means of diagnosis of smear negative pulmonary tuberculosis[1].This part of the smear-negative patients for treatment in June to 6 September the most common treatment.That the material at home and abroad：a considerable number,about 50% to 70% of sputum smear-negative cases exist[2],in control of tuberculosis in this part of the patient is the source of infection,whether treatment is worthwhile exploring the new problems,recalls three years were 676 cases of smear negative pulmonary tuberculosis,on the clinical features,diagnosis and treatment is as follows.
