摘要
目的探讨应用关节镜技术加微创锁定加压钢板治疗胫骨平台骨折的疗效。方法 2006年8月至2010年1月采用关节镜监视下骨折复位加微创锁定加压钢板治疗胫骨平台骨折19例,男10例,女9例;年龄36~83岁,平均59.5岁。致伤原因:车祸5例,高处跌落6例,自行摔伤6例,其他2例,均为闭合性骨折。按Schatzker分型,型骨折7例,型骨折5例,型骨折7例。19例患者均获随访,对其近、远期并发症进行记录统计,对膝关节功能疗效按Lysholm评分表评估。结果所有患者均无明显手术并发症,按Lysholm评分表评估膝关节功能,优16例,良2例,中1例,优良率94.7%。结论对于胫骨平台骨折患者,关节镜技术是一种有效的辅助治疗手段,微创技术及锁定加压钢板固定配合应用治疗胫骨平台骨折可达到满意的疗效。
Objective To investigate the application of treating tibia flatform factures with arthroscope technology and fixing compressed steel plate by micro-wound. Methods From August 2006 to now,treating tibia fractures by means of the fracture reposition under the monitoring of arthroscope technology plus fixing compressed steel plate by micro-wound in 19 cases. 10 cases of male and 9 female patients;aged 36 to 83 years old with an average age of 59.5 Injury reasons:5 cases by traffic accident,6 cases by the drop from height,6 cases by losing their balance,2 cases by other reason. All are closed type fractures. The type according to Schatzker : 7 cases of fracture type I , 5cases of frac- ture type Ⅱ ,7 cases of fracture type Ⅲ . 19 patients have got follow up survey with carrying on the record and statis- tics of their near and long-term complications. Evaluating the treatment effect to the knee's function according to Lysholm chart and obtaining the cases of excellent,good,medium,bad and the rate of fine. Results All the patients had no significant postoperative complications. Evaluating the knee's function according to Lysholm chart :excellent in 15 case,good in 2 cases,medium in 1 case and the fine rate is 94.7%. Conclusion For the patients with tibia frac- tures ,arthroscopic technique is an effective auxiliary treatment method,with micro wound technology and fixing corn pressed steel plate ,patients with tibia platform fractures can be treated with satisfactory results.
