摘要
目的观察经皮激光汽化减压术对腰椎力学结构的影响。方法应用半导体激光汽化仪对216例腰椎间盘突出症患者进行PLDD治疗,一年后127位患者获得随访并进行影像学检查,以CT测量术前术后治疗间隙突出间盘组织阴影变化,以X线片测量椎间隙后缘高度变化、过伸过曲位治疗椎间盘相邻椎体前后位移及成角情况,评价脊柱稳定性变化。结果 CT测量示95位患者治疗间隙突出间盘组织阴影较术前缩小,其中16位患者变化明显(≥3mm),X线片测量示所有患者治疗间隙高度没有降低,没有腰椎失稳发生。结论经皮激光减压术对腰椎力学结构没有影响。
Objective To observe the effect of percutaneous laser disc decompression (PLDD) on lumbar mechanical structure. Methods Two hundred and sixteen patients with lumbar disc herniation were treated by PLDD,and 127 of them were followed up for one year. In order to evaluate the therapy's improvement on spinal stability,a CT scan was applied to investigate shadow variations of the treated intervertebral disc; also an X-ray shot to show the effect on the height of the intervertebral space,and shifting or angulation of the two adjacent vertebrae in extend excessive and bend excessive positions. Results Intervertebral disc herniation shadow on CT graph after treatment was reduced in 95 patients,16 of which with obvious improvement(≥3mm); X-ray radiograph shows no intervertebral height decrease in treated place; lumbar destabilization wasn't detected. Conclusion PLDD didn't exert any negative effect on lumbar mechanical structure.
