痉挛型脑性瘫痪儿童足底压力分布特征分析 被引量:5

摘要 目的针对痉挛型脑性瘫痪(Spastic Cerebral Palsy,SCP)儿童下肢生物力学方面的改变,采用足底压力测量仪采集足底各区域的静态和动态压力,探讨SCP足底压力变化的特征,为今后细化SCP儿童下肢的生物力学矫形提供更加合理的干预依据。方法选择符合脑性瘫痪诊断标准的SCP儿童共39例,其中男22例,女17例,年龄5-10岁,平均(7.77±1.06)岁。入选后采用Gaitview足底压力测量仪进行测试。站立位测量左右足底前后百分比、平均压力、拇趾、2-5趾、第1跖骨、2-4跖骨、第5跖骨、足中部、足跟内侧、足跟外侧共八区静态压力参数。测动态足底压力时,儿童从平板前方2m处走向平板,先左脚踏在有效区域,后从对侧返回,右脚踏板,测动态足底压力。结果 SCP儿童静态压力以足跟及2-4跖骨压力较大,而拇趾及2-5趾则小,动态时各区压力均较静态时增加。结论 SCP儿童的足底压力分布较正常儿童不同,足底压力测量仪对SCP下肢的功能评定有很大价值。 Objective To provide more rational intervention and subdividing kind of biomechanics orthoses for the children with spastic cerebral palsy(SCP).Methods 39 SCP children,22 boys and 17 girls,5 to 10 years old(7.77±1.06),were elected and divide into experiment group and control group randomly.The Gaitview foot analyzer was used to measure the SCP child for collecting parameters,uch as the front and post percentage of both sole,average pressure,static pressure of 8 zones(hallux,No.2 to 5 toes,first metatarsal bone,No.2 to 4 metatarsal bones,fifth metatarsal bone,middle of the foot,inboard of heel,outboard of heel),and static sole area.To measure dynamic planta pressure,let the child step to the plate from 2 meters before it,tread on the active area with left foot first,then return from the other side,tread on the plate with right foot,collect the parameters of both planta pressure of all zones.Results The static pressure of heel and No.2 to 4 metatarsal bones were comparatively heavy,and that of hallux and No.2 to 5 toes were comparatively light.Dynamic pressure of each zones were all increased compare with their static pressure,respectively.Conclusion The distribrtion of planta pressure in SCP children is abnormal.The planta pressure measurement is important and valuable in functional assessment of lower limbs in SCP children.
