超声刀在甲状腺手术中的应用研究

摘要 目的:探讨超声刀在甲状腺手术中的临床意义。方法:回顾性分析2006年3月~2010年3月在我院行甲状腺手术的1435例患者的临床资料,分析手术时间、术中出血量、切口长度及并发症等情况。结果:超声刀组1123例,传统手术组312例。两组比较,超声刀组的手术时间短、术中出血量少、切口短,差异有统计学意义(P<0.05);超声刀组并发术后出血、喉返神经损伤、喉上神经损伤率较传统手术组低,差异有统计学意义(P<0.05);两组的甲状旁腺损伤差异无统计学意义(P>0.05)。结论:超声刀在甲状腺手术能显著缩短手术时间及切口长度,减少术中出血量,降低术后并发症,是安全、有效的手术方法。 Objective:To study the significance of harmonic scalpel in thyroid surgery. Methods:The clinical data of 1435 patients with thyroid diseases underwent thyroid surgery from march 2006 to march 2010 were retrospectively reviewed. The operation time, estimated blood loss in operation, the length of incision and postopera- tive complications were analyzed. Results:The harmonic scalpel group has 1123 cases, the conventional group has 312 cases, There were significantly differences between two groups in operation time, blood loss, length of incision,postoperative hemorrhage,injury of recurrent laryngeal nerve and superior laryngeal nerve ( P 〈 0.05 ). there was no any significantly difference between two groups in injury of parathyroid gland ( P 〉 0.05). Conclusions: Use of harmonic scalpel in thyroid surgery can decrease operation time, blood loss,length of incision and postoperative complications. It is safe and effective in thyroid surgery.
