眼睑黏液表皮样癌的临床病理分析

Clinical analysis and pathology for mucoepidermoid carcinoma of the eyelid

目的 探讨眼睑黏液表皮样癌的临床和病理特点.方法 回顾性分析8例眼睑黏液表皮样癌的临床病理特点,并随访其预后.结果 眼睑黏液表皮样癌临床表现类似于基底细胞癌或鳞状细胞癌,表现为眼睑糜烂溃疡或结节状肿物.8例病理标本均由黏液细胞、表皮样细胞和基底样细胞组成.8例均行显微镜下组织学控制性切除术,第一次手术后复发3例,复发患者再次手术切除后全部治愈.经病理证实理想的手术切除线为肿瘤外6～9mm.结论 眼睑黏液表皮样癌好发于老年患者,病理检查是其确诊手段.肿瘤侵袭性强,显微镜下组织学控制性切除术联合眼睑成形术是首选的治疗手段.

Objective To study the clinical characteristics, treatment, prognosis and pathology for mucoepidermoid carcinoma of the eyelid. Methods Eight cases diagnosed as mucoepidermoid carcinoma of Results Mucoepidermoid carcinoma of the eyelid appeared similar to basal cell carcinoma or squamous cell carcinoma, clinically. Tumors developed as erosion or nodular masses. By histopathology, tumor cells showed an admixture of epidermoid cells, mucus-secreting cells and basal cells. Eight cases were treated with Mohs surgery. The recurrence rate was 37.5% for the first operation. The cure rate for recurrent patients after operation was 100%. The ideal cutting line was 6-9mm from the borderline of tumor, confirmed by pathology. Conclusions Mucoepidermoid carcinoma of the eyelid is more common in the elder people. It only could be diagnosed by pathology. It is aggressive. Mohs surgery and eyelid reconstruction are the main treatment.
