摘要
目的 探讨脑出血早期血肿扩大的发生率、发病机制、影响因素及防治措施. 方法 回顾性总结了76例经头颅CT扫描证实的脑出血早期血肿扩大患者的临床和CT资料. 结果 脑出血早期血肿扩大的发生率为14.1%（76/538）,早期血肿扩大的发生时间多数在发病后24 h以内（86.8%）.经内科保守治疗30例,死亡8例,病死率19.5%;颅内血肿微创清除术20例,死亡4例,病死率16.7%;开颅清除血肿18例,死亡4例,病死率36.4%.存活患者均遗留有不同程度的后遗症. 结论 脑出血早期血肿扩大是导致病情加重与死亡的重要因素.早期血肿扩大与血肿形态、出血部位、早期血压过高、凝血功能障碍、不适当应用脱水剂等有关.
Objective To investigate the risk factors of early enlargement of hematoma in cerebral hemorrhage incidence. Methods A retrospective summary of 76 cases of cranial CT scans confirmed the early enlargement of hematoma in cerebral hemorrhage. Results Thirty cases accepted conservative medical management,and 8 of them died.The fatality rate was 19.5%.Twenty cases accepted mini-invasive evacuation of hematoma and 4 of them died.The fatality rate was 16.7%.Eighteen cases accepted clearance of hematoma by crainiotomy and 4 of them died.The fatility rate was 36.4%.There were sequelae in survivals.The incidence of early hematoma enlargement was 14.1%. Conclusions Early enlargement of hematoma is a key factor to aggravating and death.Early enlargement of hematoma is related to shape and position of hematoma,hyperpiesia,coagulation disorders and inappropriate usage of dehydrant.
