摘要
目的探讨接触式内镜辅助诊断鼻咽癌的应用价值。方法对门诊28例可疑鼻咽癌患者采用接触式内镜检查鼻咽部,采集图像并在内镜指导下取材活检,将接触式内镜诊断结果与病理诊断结果进行对比分析。结果 28例中病理确诊鼻咽癌20例(71.4%),良性病变8例(28.6%);20例鼻咽癌中接触式内镜诊断出15例(75%),8例良性全部诊断正确(100%)。接触式内镜诊断鼻咽癌的敏感性和特异性分别为75%和100%,总诊断准确率为82.1%,两种诊断一致性检验的Kappa值为0.63(P<0.05)。结论接触式内镜对鼻咽癌具有较好的辅助诊断价值。
【Objective】 To evaluate contact endoscopy for assistant diagnosis of nasopharyngeal carcinoma.【Methods】To examine 28 cases which were suspected nasopharyngeal carcinoma with contact endoscopy.Biopsy of the areas under examination was performed.The contact endoscopic results were correlated with the corresponding histological sections of the biopsy tissues.【Results】Of the 28 examined specimen,20（71.4%） showed Squamous cell carcinoma in the histological analysis.Benign change was revealed in 8（28.6%）.Using contact endoscopy,15 of 20（75%） carcinomas were correctly categorized while the accurate identification of the benign findings is 100%（8 of 8）.The sensitivity and specificity of endoscopic findings was 75% and 100%,diagnostic accuracy is 82.1%.Kappa reliability coefficient =0.63（P〈 0.05）.【Conclusions】Contact endoscopy is useful for assistant diagnosis of nasopharyngeal carcinoma.
