胃镜下钛夹、热极凝固及注射治疗急性非静脉曲张上消化道出血的疗效比较

Comparison of the curative effect of astroendoscopic titanium clips,heat probe coagulation and injection adrenaline for acute nonvariceal upper gastrointestinal bleeding

目的评价胃镜下钛夹、热极凝固及注射治疗急性非静脉曲张上消化道出血的疗效。方法经胃镜确诊为急性非静脉曲张上消化道出血85例,随机分成钛夹组(n=29)、热极凝固组(n=28)和注射组(n=28)对出血灶进行治疗,评价3种治疗方法的止血率和再出血率的差异。结果钛夹、热极凝固及注射组首次止血率分别为100.00%、96.43%、92.86%,差异无显著性(P>0.05)。钛夹组和热极凝固组各有1例发生再次出血,再出血率分别为3.45%、3.70%,两组再出血率差异无显著性(P>0.05)。注射组中有6例发生再出血,再出血率23.08%,高于钛夹组和热极凝固组,差异有显著性(P<0.05)。结论胃镜下钛夹与热极凝固治疗非静脉曲张上消化道出血疗效相当,优于注射治疗。

【Objective】To evaluate the curative effect of gastroendoscopic titanium clips,heat probe coagulation and injection adrenaline for acute nonvariceal upper gastrointestinal bleeding（ANVUGIB）.【Methods】A total of 85 cases of ANVUGIB were randomly divided into titanium clips group（n =29）,heat probe coagulation group（n =28） and injection group（n =28） for treatment of bleeding.The rate of curative effect of hemostatic and rehaemorrhagia was compared.【Results】The rates of initial hemostasis in titanium clips group,heat probe coagulation group and injection group were 100.00%,96.43% and 92.86% respectively,there was no significant difference among them.One case of rebleeding was in the titanium clips group and one in heat probe coagulation group.The rehaemorrhagia rates were 3.45% and 3.70% respectively.6 cases of rebleeding were found in the injection group and the rehaemorrhagia rate was 23.08%,which was significanly higher than that in the other two groups.【Conclusions】The curative effect of titanium clips is the same and heat probe coagulationare,either of them is superior to injection in the treatment of acute nonvariceal upper gastrointestinal bleeding.
