药物性肝病临床分型和病理相关性分析

Correlation between clinical classification and histopathology in drug-induced liver injury

目的了解药物性肝病临床分型和病理学特点之间的相关性。方法对106例药物性肝病患者进行临床分型,比较不同分型患者血生化、肝组织病理学特征,并进行相关性分析。结果肝细胞损伤型和胆汁淤积型患者分别占总数的58.5%和38.7%,两者ALT和ALP水平差异有统计学意义,但GGT和总胆红素水平差异无统计学意义,且在病理学特点方面差异也无统计学意义。结论目前的临床分型标准与病理学特征并不十分吻合,需要进一步研究完善。

Objective To analyse the clinical type and pathologic characters of drug-induced liver injury.MethodsOne hundred and six patients with drug-induced liver injury were enrolled.Clinical data including liver function and histology of liver biopsy were collected and analysed.Results The proportion of hepatocellular type and cholestatic type was 58.5% and 38.7% respectively.There were significant differences in ALT and ALP level between different types,but no significant differences were found in GGT,total bilirubin level and pathologic characters.Conclusion Clinical classification of drug-indued liver injury did not coincide well with pathologic characters and some modifications are necessary.
