乳房外Paget病的皮肤外科修复治疗

The Dermatologic Prosthetic Surgery Treatment for the Extramammary Paget's Disease

目的:探讨乳房外Paget病的皮肤外科修复治疗效果。方法:术前6例患者均经病理证实为乳房外Paget病,术中据冰冻切片结果指导扩大切除范围,阴囊皮瓣、邻位旋转皮瓣或游离皮片植皮修复缺损。结果:6例患者切口愈合良好,局部形态及功能满意;随访6个月～2年,局部皮损均无复发,淋巴结转移1例。结论:乳房外Paget病通过皮肤外科扩大切除并修复治疗,既能完好地修复大面积的皮损,又能恢复其外形及功能。

Objective：To explore the effectiveness of dermatologic prosthetic surgery treatment for the extramammary Paget＇s disease. Methods：Six male patients with extramammary Paget＇s disease confirmed by pathology were enrolled in this study.Microscopic examination of snap-frozen section was performed to guide the excision range.Skin flaps from the scrotum,free skin grafts or rotation skin flaps from adjacent areas were transplanted to repair the operative wound.Results：The post-operation wound healing was satisfactory morphologically and functionally in all of the 6 cases.Lymph node metastasis was observed in one case,but no local recurrence was observed in any of the cases in follow up after 6 months to 2 years.Conclusion：Dermatologic prosthetic surgery treatment is a successful treatment for the extramammary Paget disease,which can repair the large skin lesions and recover their function.
