关节镜结合射频治疗仪治疗膝关节腔内良性肿瘤的体会

Radiofrequency ablation technique in arthroscopic treatment for intra-articular neoplasm of the knee

目的介绍应用关节镜结合射频治疗仪切除膝关节腔内不同类型良性肿瘤的治疗方法及体会。方法对13例膝关节内肿瘤的患者进行关节镜检并切除治疗,采用整个切除或分步咬取方法完整切除肿瘤,结合射频治疗仪清理肿瘤基底及外周组织,观察手术疗效,以Lysholm评分系统评估关节功能。结果本组所有患者的病理结果均为良性病变:黄色素瘤5例,囊肿3例,软骨瘤2例,滑膜血管瘤2例,脂肪瘤1例。所有患者均获随访,随访时间2～6年,无一例复发,关节功能由术前(76.5±4.5)分提高到(96.7±3.8)分,差异有统计学意义(t=4.874,P<0.05)。结论应用关节镜结合射频治疗仪对膝关节内良性肿瘤进行切除治疗具有创伤小、康复快、切除彻底、疗效确切等优点,值得临床推广应用。

Objective To introduce the radiofrequency ablation technique in arthroscopic treatment and excision experience of intra-articular neoplasm of the knee.Methods 13 cases of tumor diagnosed in patients had arthroscopical excision,using the entire removal or sub-step tumor resection,and combined radiofrequency ablation technique cleaning-up basal tissue and peripheral tissue.The function of involved knee was evaluated with Lysholm scores after operation.Results Pathological findings were benign lesions：five cases of yellow pigment tumors,three cases of cysts,two cases of osteochondroma,two cases of synovial hemangioma,and one case of lipoma.There was no recurrent case.All cases were followed up for 2-6 years.The Lysholm score increased from （76.5±4.5） to （96.7±3.8）,and the difference was statistically significant （t=4.874,P〈0.05）.Conclusions Radiofrequency ablation technique in arthroscopic treatment of intra-articular benign tumor is an effective treatment and has the worth of clinical application.
