实时三维超声心动图评估儿童川崎病左室收缩不同步的应用价值 被引量:1

摘要 目的探讨实时三维超声心动图对儿童川崎病(KD)左室收缩不同步性评估的应用价值。方法收集KD患者60例,健康志愿者40名。分析所有患者的二维经胸超声心动图Simpson法所测射血分数值并与三维超声心动图整体射血分数值比较。实时三维超声心动图分析患者左室17节段时间-容积曲线图,并与正常对照组对比。结果 KD患者二维Simpson法所测射血分数值与三维测量值呈中等程度相关。KD患者17节段时间-容积曲线呈方向不同的波形,其排列紊乱、不规则,到达最小容积的时间较离散。结论 17节段时间-容积曲线可作为评价左室心肌收缩不同步性的有效指标。 Objective To explore application value of real-time three-dimensional echocardiography in assessment of left ventricular asynchrony in children with Kawasaki disease(KD). Methods Sixty children with KD were analyzed retrospectively,and 40 healthy volunteers were enrolled as control group.The two-dimensional echocardiographic features and ejection fractions measured by Simpson's method of 60 cases were analyzed and compared with the global ejection fractions measured by three-dimensional echocardiography.The regional volume-time curves of 17 segments were acquired automatically by real-time three-dimensional echocardiography and compared to the control group. Results In 60 cases,the ejection fractions measured by Simpson's method correlated moderately with the global ejection fractions measured by real-time three-dimensional echocardiography;and the regional volume-time curves of 17 segments arranged in disorder of different directions;and the time to the point with minimal systolic volume was discrete. Conclusion The regional volume-time curves of 17 segments are important in evaluating the left ventricular systolic synchrony.
