上颌窦提升术在种植修复中的应用及长期疗效评估 被引量:8

Clinical Application and a Long Term Clinic Follow-up Study of Maxillary Sinus Lifting,Bone Graft and Simultaneous Placement of Implant

摘要 目的评价开放式和闭合式上颌窦提升术应用于种植修复的长期疗效。方法 46例患者,共51侧68颗上颌后牙缺失,接受2种上颌窦提升术并同期植入Rep lace种植体,其中21例26侧行开放式手术植入41颗种植体,25例25侧行闭合式手术植入27颗种植体。随访15~68个月,行疗效评估。结果随访期间,接受上颌窦开放式提升术的21例患者41颗种植体有2颗松动,1颗发生进行性骨吸收,接受闭合式手术的25例患者27颗种植体未见松动、脱落。X线检查显示上颌窦开放式提升术后窦底平均抬高(4.90±0.17)mm,闭合式提升术后窦底平均抬高(3.70±0.62)mm。结论开放式提升术和闭合式提升术均可取得较高的成功率,关键在于适应证的选择和掌握相关手术技巧。 Objective To evaluate the clinical application of maxillary sinus lifting techniques and its long term clinical effect.Methods Fifty-one sites of 46 patients undergone two kinds of maxillary sinus lifting techniques and simultaneous implant replace placement were analyzed from 2003 to 2008.All patients were followed-up from 15 to 68 months,the mean time was 39.4 months.Results After the follow-up of 46 patients,1 patient with 2 implants were found loosen,and 1 patient with 1 implant was found progressive bone loss using the maxillary sinus augmentation elevation,and the maxillary sinus floor were elevated(4.90±0.17) mm.All implants were well applied the internal maxillary sinus floor elevation,and the maxillary sinus floor were elevated(3.70±0.62) mm.Conclusion Both kinds of maxillary sinus lifting techniques were predicable to implant rehabilitation,it's crucial to handle the indication and operation skill.The internal maxillary sinus floor elevation was easy,target of simple,the trauma was small and the rate of success was high,patients were more easy to satisfy.
