摘要
目的观察冠状动脉介入治疗(PCI)对冠心病患者心电图QT离散度(QTd)及矫正QT离散度(QTcd)的早期影响。方法选择127例冠心病患者,按临床最终诊断分成稳定型心绞痛(SA)组、不稳定型心绞痛(UA)组、急性心肌梗死(AMI)组,分别测定其PCI术前和术后早期的QTd、QTcd,与排除冠心病者进行比较研究。结果 SA组患者PCI术后早期QTd、QTcd显著增大,UA组患者PCI术后早期QTd减小,AMI组及对照组则变化不明显。结论 PCI术对冠状动脉及自主神经系统产生刺激,使QTd增大,是PCI早期作用的主要机制;PCI对QTd、QTcd的影响与冠状动脉基础病变有关。
Objective: To observe the early effects of PCI on ECG QT dispersion(QTd) and corrected QT dispersion(QTcd)in patients with coronary heart disease.Methods: 127 cases of coronary heart disease were choosedn and divided into stable angina(SA) group,unstable angina(UA) group,acute myocardial infarction(AMI) group according to the clinical final diagnosis,then the early QTd and QTcd were determined respectively before PCI and after PCI,compared with those excluded coronary heart disease.Results: After PCI,early QTd and QTcd were significantly increased in the patients of SA group,and early QTd decreased in UA patients.AMI group did not change significantly compared with the contrast group.Conclusion: The primary mechanism for the role of early PCI is that PCI surgery can stimulate coronary artery and the autonomic nervous system to increase QTd;PCI impact on QTd,QTcd is related to the basic pathologic change of the coronary artery.
