Tei指数监测柔红霉素对急性淋巴细胞白血病患儿心功能的影响 被引量:4

摘要 目的:探讨心肌性能指数(myocardial performance index,MPI,又称Tei指数)在评价柔红霉素(daunorubicin,DNR)对急性淋巴细胞白血病(ALL)患儿心功能早期损害方面的临床应用价值。方法:测定并比较22例急性淋巴细胞白血病患儿DNR化疗前和化疗后6个月、12个月常规超声心动图参数及Tei指数,同时以25例正常健康体检儿童为对照组。结果:急性淋巴细胞白血病患儿化疗后6个月左心室Tei指数明显高于化疗前组及对照组(P<0.01),化疗前组与对照组左心室Tei指数差异无显著性(P>0.05),ALL患儿化疗后6个月左室射血分数(LVEF)、E/A比值与化疗前比较,差异均无显著性(P>0.05)。结论:Tei指数能更早、更敏感地评价DNR对急性淋巴细胞白血病患儿心脏功能损害。 Objective:To explore the clinical value of Tei index in assessing daunorubicin on cardiac function damage in children with acute lymphocytic leukemia.Methods:Twenty-two children with acute lymphocytic leukemia treated with daunorubicin chemotherapy were examined by echocardiography and Tei index before chemotherapy and six months and twelve months after chemotherapy.And 25 normal children were used as controls.Results:Left ventricular Tei index were significantly higher after of six months chemotherapy(P 0.01),and Tei index with acute lymphocytic leukemia before chemotherapy was not significantly higher than that of the control group(P 0.05).While Left ventricular ejection fraction(LVEF),the E/A ratio showed no significant difference between the group(sP〈0.05).Conclusion:Compared with conventional echocardiography,Tei indexes are more sensitive in detecting abnormalities of cardiac function in children with acute lymphocytic leukemia treated with daunorubicin.
