多层螺旋CT(MSCT)对健康成人第二肩关节间隙的测量研究 被引量:12

摘要 目的研究第二肩关节间隙的多层螺旋CT(MSCT)测量方法并统计其正常范围。方法使用GELigtspeed16层螺旋CT对30~60岁的健康成人男、女各30例,共120个肩关节进行扫描,将原始数据传至MSCT工作站进行VR、MPR、MIP等后处理,然后分别在VR、MPR、MIP等图像上观察、测量肩峰外段前下缘和与之平行的肱骨头切线之间的距离"峰肱距"(h1),肩峰最外端和与之对应的肱骨头结节间沟之间的距离"峰沟距"(h2)。结果 h1、h2在VR和MPR像上测量结果较为接近、MIP像测量结果差异稍大,但h1、h2的95%参考范围无论是VR、MPR像还是MIP像都分别为6.62mm~9.85mm和11.53~17.86mm。结论 h1、h2均能较准确的反应第二肩关节间隙的骨性距离,因MSCT具有VR、MPR、MIP等多种后处理工具,因此,用MSCT测量更具优势且准确性更高,对指导临床诊断及治疗有重要意义。 Objective To study the second shoulder joint space of multislice spiral CT(MSCT) measurement methods and statistics of their normal range.Methods The use of GE Ligtspeed 16-slice spiral CT in 20-70-year-old healthy adults men and women in all 30 cases,a total of 120 shoulder joints scan the raw data transmitted to MSCT workstation VR,MPR,MIP and other post-processing,and then respectively VR,MPR,MIP images such as observation and measurement the distance between the anterior margin of acromion outer segments and parallel to with the humeral head tangent "peak humeral distance"(h1),between the outer end of the acromion and the corresponding of the humeral head of nodules sulcus "peak sulcus distance"(h2).and the outer end of the humerus between the first node the distance between the "peak away from the ditch"(h2) Results h1,h2 in the VR and MPR images in measurement results is closer,slightly as the difference in the MIP in measurement results,However,h1 and h2 95% of the reference range of either VR,MPR or MIP have respectively to 6.62mm^9.85mm and 11.53~17.86mm.Conclusion Both h1 and h2 can correctly reflect the bone distance of the second shoulder space,because of MSCT with VR,MPR,MIP and other postprocessing tools,therefore,with the advantages and the higher accuracy of MSCT measurements which is significantly meaningful for guiding clinical diagnosis and therapy.
