全身弥散加权成像在恶性肿瘤骨转移早期诊断中的应用价值 被引量:5

摘要 目的探讨全身弥散加权成像在恶性肿瘤骨转移早期诊断中的应用价值。方法对健康志愿者10名及病理学检查证实为恶性肿瘤患者83例行全身弥散加权成像(WB-DWI)检查,并于同一时间对可疑部位行常规磁共振成像(MRI)检查,必要时增强扫描,诊断结果与临床综合评价结果进行比较,分别比较两者所显示的病例数。结果 83例患者中34例经临床综合评价为骨转移瘤。常规MRI共检出33例骨转移瘤患者,漏诊1例胸骨转移瘤患者;WB-DWI共检出38例骨转移瘤患者,漏诊1例前列腺癌骨盆成骨性移瘤患者,误诊5例。结论 WB-DWI在恶性肿瘤骨转移早期诊断中具有一定价值,结合常规MRI检查可进一步明确诊断。 Objective To explore the clinical value of whole-body magnetic resonance diffusion weighted imaging (WB-DWI) in the early detection of malignant tumor bone metastasis. Methods Ten healthy volunteers and 83 patients with pathologically validated malignant tumors were underwent WB-DWI, meanwhile, conventional magnetic resonance imaging (MRI) over suspicious sites was performed, with enhanced scanning given when necessary. The detecting rates of bone metastasis by WB-DWI and conventional MRI were compared with final clinical diagnosis. Resuits Thirty-four patients were diagnosed with bone metastasis clinically. Thirty-three patients were diagnosed with bone metastasis by conventional MRI, and one patient with sternum metastasis was missed. There were 38 patients diagnosed with bone metastasis by WB-DWI, one case with osteogenie metastasis from prostate cancer in pelvis was missed, and five cases were misdiagnosed. Conclusion WB-DWI has a certain value on early diagnosis of bone metastatic lesions, and definite diagnosis may be achieved when combining WB-DWI with conventional MRI.
