热疗联合胸腔内注射顺铂治疗恶性胸腔积液的疗效观察

Clinical observation on the efficacy of intrapleural perfusion of cisplatin combined with thermotherapy in treating malignant pleural effusion

目的观察胸腔注射顺铂联合热物理疗法对恶性胸腔积液的疗效。方法将有胸闷、气促症状的恶性胸腔积液患者39例随机分为对照组(胸腔置管引流胸腔积液后注入顺铂75 mg/m2,分2次)和治疗组(胸腔置管引流胸腔积液后注入顺铂75 mg/m2,分2次),并给予热疗。结果胸腔穿刺引流术成功率100%,治疗后评估,对照组19例总有效率(CR+PR)为47%,治疗组20例总有效率(CR+PR)为82%;2组均无明显骨髓抑制及肝肾功能异常。结论胸腔置管引流联合胸腔注入顺铂及热疗对恶性胸腔积液的治疗安全、有效;胸腔积液引流后胸腔注入顺铂联合热疗比单纯胸腔内注入顺铂更能有效控制恶性胸腔积液。

Objective To evaluate efficacy of intrapleural perfusion of cisplatin combined with thermotherapy in treating malignant pleural effusion（MPE）.Methods Thirty-nine patients with symptomatic MPE were randomly assigned to two groups.The control group（19 patients） was only treated with intrapleural perfusion of cisplatin after drainage of the pleural effusion.The treated group（20 patients） received thermotherapy combined with intrapleural perfusion of cisplatin after drainage of the pleural effusion.Results Success rates of pleural indwelling catheterization was 100%.The total effective rates for the treatment group（82%） were significantly higher than that for the control group（47%）,All patients tolerated the therapy well and there were no significant toxicities.Conclusion Intrapleural perfusion of cisplatin combined with thermotherapy in treating MPE is a safe,effective method.Intrapleural perfusion of cisplatin combined with thermotherapy is more effective in controlling MPE than monotherapy of intrapleural perfusion of cisplatin.
