带蒂股前外侧皮瓣的临床应用

Pedicled Anterolateral Thegh Flap of the Clinical Application

目的报道带蒂股前外侧皮瓣顺行修复股骨粗隆部褥疮及逆行修复膝部及小腿中上段严重软组织缺损中的临床应用。方法对4例股骨粗隆部褥疮及9例膝关节附近软组织缺损的病例,分别采用顺、逆行股前外侧皮瓣修复,皮瓣面积最大24cm×13cm,最小6cm×9cm。结果 12例皮瓣完全存活,1例部分存活,随访12～36个月,皮瓣质地优良,外观及功能满意。结论带蒂股前外侧皮瓣较好的解决了髂部、膝部及小腿中上段软组织缺损修复难的问题,该皮瓣切取方便,旋转弧度大,面积大,能满意地修复髂部、膝部及小腿上段软组织缺损。

Objective To report the vascular pedicled andicled anterolateral thigh flap in repairing the serious slft tissue defect of the great trochanter region pressure sores the knee and upper calf in the clinical application.Methods 4 pateins of the great trochanter region pressure sore and 9 patinets with serious soft tissue defect of the knee and upper alf were treated with anterolateral thigh flap,the flaps meaured 24cm×13cm to 6cm×9cm.Results The flaps totally survived in 12 patients,the other was patially survived,the color and texture of the flaps were good.Thir apperance and function were satisfactory after 12～36 months flollow-up.Conclusuion It is settled the problem of treated with the vessel pedicled anterolatral thigh flap repaired the serious soft tissue defects of the great trochanter region pressure soresv、the knees and upper calf.The flap is dissected easily and has a large rotary arc,this was satisfactory in repairing of the soft tissur defects around the great trochanter region pressure sores、the knees and upper calf.
