持续气道正压通气治疗对阻塞性睡眠呼吸暂停低通气综合征患者胰岛素敏感性的影响

Effects of Continuous Positive Airway Pressure Treatment on Insulin Sensitivity in Patients with Obstructive Sleep Apnea Hypopnea Syndrome

摘要 目的探讨持续气道正压通气(CPAP)治疗对阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者的胰岛素敏感性指数(ISI)的影响。方法选择30例确诊OSAHS患者,通过3个月的CPAP治疗,观察治疗前后空腹血糖(FBG)、空腹胰岛素(FIns)、空腹C-肽(FCP)、糖化血红蛋白A1(CHbA1C)及胰岛素敏感指数(ISI)的变化。结果在BMI无明显改变的情况下,经过3个月CPAP治疗后FBG、FIns、FCP、HbA1C较治疗前降低(P<0.05),ISI较治疗前明显增加(P<0.05)。结论 CPAP治疗可显著改善OSAHS患者的ISI,降低血糖,OSAHS可能是引起糖耐量减低和胰岛素敏感性下降的重要因素。 Objective To investigate the effects of continuous positive airway pressure(CPAP)treatment on insulin sensitivity in patients with obstructive sleep apnea hypopnea syndrome(OSAHS).Methods Thirty patients with OSAHS were recruited.The insulin sensitivity index(ISI) was used to reflect IR and measured before and after 3-months CPAP treatment,and so were the other blood parameters(including fasting blood glucose,fasting blood insulin,fasting C-peptide,HbA1c,insulin sensitivity index) in both genders.Results The levels of FBG,Fins,FCP,HbA1c,TG were all significantly lower at the end of 3-month CPAP treatment than those before the treatment,the ISI was also significantly increased following 3-months CPAP treatment than that before treatment(P〈0.05).Conclusion CPAP treatment could significantly increase ISI in OSAHS patients,decrease blood glucose;OSAHS maybe the important factors contributing to the impaired glucose and ISI declined.
