2型糖尿病肾病与糖尿病视网膜病变的相关性与平行性研究

A Study of the Associativity and Parallelism between Type 2 Diabetic Nephropathy and Diabetic Retinopathy

目的探讨不同阶段的2型糖尿病肾病(DN)与糖尿病视网膜病变(DR)之间相关性与平行性的关系。方法 330例2型糖尿病患者分为无白蛋白尿组(DM0)、微量蛋白尿组(DM1)、临床蛋白尿组(DM2),均进行眼底、尿微量白蛋白排泄率(UAER)、糖化血红蛋白、胰岛功能、尿素氮、肌酐、血脂、空腹、餐后2小时血糖等检查,比较不同组别间糖尿病视网膜病变的发生率及与视网膜病变的严重程度是否平行,并对检测结果进行比较分析。结果 DR的发生率在临床蛋白尿组最高,为86.9%,其次是微量蛋白尿组,为57.5%,无白蛋白尿组DR的发生率最低,为16.4%;DN的发生与病程关系密切。结论随着DN的加重,DR的发病率也呈明显增高的趋势,提示了两者之间的密切相关性,但DN与DR之间的严重程度并不完全平行。DN的发生与病程密切相关。

Objective To investigate the relationship of associativity and parallelism between diabetic nephropathy and diabetic retinopathy in different stage.Methods 330 type 2 diabetes mellitus patients were divided into normal group（DM0）,microalbuminuria group（DM1） and macroalubuminuria group（DM2）.All were undergone eyeground,UAER,HbA1c,BUN,Cr,blood fat,FBG,PBG examinations.The incidences of diabetic retinopathy and severity of these three groups were compared in parallel.The results were analyzed and compared.Results The incidence of DR was the highest in macroalbunuria group,which was 86.9%.It was followed by microalbuminuria group,which was 57.5%.The incidence of DR was the lowest in normal group,which was 16.4%.The relationship of the occurrence and the course of DN was intimate.Conclusions As DN aggravates,the incidence of DR has tendency of significant increase,indicating their intimate associativity.However,the severity between DR and DN isn＇t totally parallel.The occurrence of DN is related intimately to the course.
