局麻下无张力疝修补术在腹股沟疝治疗中的优势

Advantages of tension-free herniorrhaphy in patients with inguinal hernia under local anesthesia

目的探讨局麻下无张力疝修补术治疗腹股沟疝的优势。方法随机对我科2005年10月至2009年2月间收治的116例腹股沟疝患者分别采用局麻和硬膜外麻醉行无张力疝修补术,并对两组的手术时间、术后禁食时间、住院天数、住院费用和术后并发症进行研究。结果局麻组住院费用明显低于硬膜外组(P<0.001),手术时间和住院天数明显缩短(P<0.001),而术后并发症两组间无差异(P>0.05)。所有患者切口均甲级愈合并痊愈出院,随访6个月以上无复发。结论局麻下无张力疝修补术治疗腹股沟疝经济方便,安全有效,复发率低,并发症少。

Objective To evaluate the advantages of tension-free herniorrhaphy in patients with inguinal hernia under local anesthesia. Methods One hundred and sixteen patients with inguinal hernia were randomly divided into two groups （local anesthesia group and epidural anesthesia group） from October 2005 to February 2009 in our department. All of them were underwent tension-free herniorrhaphy. Operation time, hospital days, hospital charges and postoperative complications were compared between the two groups. Results Hospital charges in local anesthesia were much cheaper than that in epidural anesthesia group （P 〈 0.001 ）. Anesthesia plus operation time and hospital clays in local anesthesia were much shorter than that in epidural anesthesia group（P 〈 0. 001 ）. There was no difference of the postoperative complications between the two groups（P 〉0.05）. All patients were cured, no postoperative chronic pain, no incision infection, no recurrence during at least 6-month follow-up period. Conclusions Tension-free herniorrhaphy under local anesthesia is economic, sate, reliable and lower incidence rate of postoperative complications.
