硅胶管重建颅内静脉窦的实验研究 被引量:2

An experimental study of reconstruction of intracranial venous sinus using silicone tube

摘要 目的 为今后临床上更安全有效而广泛地重建颅内静脉窦进行初步尝试。方法 全麻下开颅切除8 只犬上矢状窦后, 以硅胶管为替代材料, 采用置管法和吻合法重建上矢状窦。通过数字减影矢状窦造影和经颅多普勒(TCD) 分别检测术前并比较术后1、2、4 和8 周重建上矢状窦的通畅性, 同时观察了组织学和超微结构。结果 矢状窦造影和TCD 证实5 只通畅,1 只不全阻塞,1 只仅近端堵塞, 另1 只术后丢失。术后除1 只呕吐外均无任何神经系统症状。组织学及超微结构表明术后8 周硅胶管和上矢状窦吻合处已有内皮细胞增生、爬行, 表面光滑未见血栓形成, 重建矢状窦通畅。结论 硅胶管作为重建颅内静脉窦的人工材料是可行的, 有很好的生物相容性, 无血栓源性, 再通性强, 无任何不良反应及毒副作用, 至少近斯效果良好, 待临床Ⅰ期实验。 Objective To perform reconstruction of intracranial venous sinus for safely, effectively and extensively in clinical use. Methods The superior sagittal sinus(SSS) was reestablished on 8 dogs under the methods of the tube insertion as well as the end to end anastomosis with a thin walled silicone tube, after the SSS were resected.The patency of reconstruction of SSS were observed by digital substruction angiography and trans cranial Doppler(TCD) ultrasonic pre and post operation 1,2,4 and 8 weeks, respectively. The histology and ultrastructure were observed under the light and electron microscope. Results The result of the experiment demonstrats that 5 of them are patent, one is narrowed, another is completely obliterated in the proximal of anastomosis site, one dogs lost one week after operation.No dog had any additional neurological deficits, side effect and toxic action.The result of histology and ultrostructure studies show vascular endothelial proliferation no thrombosis occured in the inner surface at the anastomosis site and in the luman of silicone tube as well as sagital sinous on the eight weeks. Conclusion This experiment confirms that it is available for the techniggue of intracranial venous sinus reestablished with a thin walled silicone tube as an artificial subsitiute which has good biological compatibility: non thrombogenic and high patency is not only very simple and effective but practicable technigue in the clinic, at least short term (eight weeks) patency is satisfactory. The report emphasizes that phase I clinical trial of the reconstruction of intracranial venons sinus with silicone tubing should be taken further research.
