牵引下与常规经皮腰椎间盘摘除术疗效观察

Study of Curative Effect of Automatic Percutaneoous Lumbar Discectomy during Traction

分析牵引下与常规经皮腰椎间盘摘除术的疗效。采用牵引下与常规经皮腰椎间盘摘除术,分别治疗28例腰椎间盘突出症患者,两组对比观察,髓核摘除量牵引组(均值标准差2.73±0.67)明显高于常规组(均值标准差1.76±0.65)。两组患者均经一年以上随诊,优良率牵引组96.4％;常规组71.4％。另外,我们曾在尸体腰椎上牵引穿刺,实验发现,穿刺针不易损伤神经根,因此我们认为,牵引下经皮腰椎间盘摘术安全可靠,简单易行,可明显提高手术疗效。

We have applied automatic percutoneous lumbar discectomy (APLD) and APLD during spinal column trac-tion technique in 28 patients. The mean of the volume of nucleus pulpous excised in patients of APLD during traction group is 2.73 ?.67, while those without traction is 1.76 ?.65.Each patient was followed-up more than one year.Good re-sult rate increased from 71.4% to 96.4%. We found the combination traction with APLD improved retraction and exci-sion of nucleus pulpous when it was not calcified.Experiment showed that there were no nerve roots entxapments while punctuated lumbar column was pulled. The procedure of APLD during traction technique is simple and safe.
