单光子发射型电子计算机断层扫描测定肾小球滤过率与传统方法比较研究

Measurement of glomerular filtration rate using SPECT and comparison with traditional methods

目的：为评估多种推测肾小球滤过率（ＧＦＲ）方法的敏感性和准确性提供科学依据。方法：对５８例慢性肾脏病患者采用单光子发射型电子计算机断层扫描装置（ＳＰＥＣＴ）进行９９锝二乙三胺五醋酸盐（９９ｍＴｃ－ＤＴＰＡ）肾动态显像测定ＧＦＲ，并与同时测定的血清尿素氮（ＢＵＮ）、肌酐（Ｓｃｒ）及计算内生肌酐清除率（Ｃｃｒ）进行比较分析。结果：上述指标均能反映肾小球滤过功能，但９９ｍＴｃ－ＤＴＰＡ肾动态显像测定的ＧＦＲ在敏感性和准确性明显优于其它方法。结论：用ＳＰＥＣＴ进行９９ｍＴｃ－ＤＴＰＡ肾动态显像测定ＧＦＲ是目前临床上了解肾小球滤过功能较好方法之一，但需要昂贵仪器。
