胃癌合并非肥胖型2型糖尿病患者行胃转流术的护理

Nursing of Gastric Bypass in Patients with Gastric Cancer with Non-obese Type 2 Diabetes

目的探讨胃癌合并非肥胖型2型糖尿病患者行胃转流术的护理方法。方法对103例胃癌合并非肥胖型2型糖尿病行胃转流术患者,术前进行护理评估,给予心理护理,指导合理膳食;术后注意观察病情,监测和控制血糖,加强管道护理,给予合理的营养支持,预防并发症的发生等。结果术后6个月,103例患者中有82例不需药物或饮食控制,空腹血糖(fasting plasma glucose,FPG)在正常水平以内,有效率为79.6%。余21例患者FPG仍高于正常水平,需药物或饮食控制血糖,但药物剂量较术前减少,且空腹血糖水平较术前有所下降。结论对胃癌合并非肥胖型2型糖尿病行胃转流术患者给予监测和控制血糖、严密观察病情等整体护理,为手术顺利进行提供了保证,并减少了术后并发症的发生。

Objective To explore the nursing measures of the non-obese type 2 diabetic patients with gastric cancer undergoing gastric bypass.Methods A total of 103 non-obese type 2 diabetic patients with gastric cancer undergoing gastric bypass received preoperative nursing assessment,psychological nursing and guidance of reasonable diet.After surgery,more concerns should be attached to observing patient＇s condition,monitoring and controlling blood sugar,enhancing pipeline nursing,giving a reasonable nutritional support to prevent complications,etc.Results Totally 82 out of the 103 diabetics＇ fasting plasma glucose（FPG） were controlled under normal standards without medical or dietetic control 6 months after the operations and the cure rate of 79.6%.Though the FPG levels of the rest 21 diabetics were still above the normal standard and medical or dietetic control were still necessary,their medicine dose and FPG standards were lower than those before the operations.Conclusion By closely observing patient＇s condition and monitoring and controlling blood sugar of the gastric cancer and non-obese type 2 diabetics undergoing gastric bypass operation,the operations will be conducted more smoothly with less postoperative complications.
