摘要 目的探讨TTF-1、CK7在胸水细胞块中鉴别肺腺癌中表达的敏感性和特异性及在肺腺癌诊断中的应用。方法临床资料和影像学资料诊断为肺癌的病人,胸水离心涂片中见到癌细胞,胸水再离心制成细胞块,HE显微镜观察有肿瘤细胞,进行免疫组化Max Vision法检测TTF-1及CK7等抗体。结果10例肺癌中有7例肺腺癌和1例肺小细胞癌都表达TTF-1、CK7;2例肺鳞癌中TTF-1、CK7不表达。结论 TTF-1、CK7在肺腺癌中表达有较高的敏感性和特异性,胸水细胞块诊断后,可作为鉴别肺腺癌的可靠标记抗体。 Objective To investigate the specificity and sensitivity of TTF-1 and CK7 in pulmonary adenocarcinoma of pleural fluid Cell Blocks and to investigate the combined applications.Methods The cancer diagnosed by clinic symptom and pulmonary video,had found tumour cells in pleural fluid,we made pleural fluid Cell Blocks using the pleural fluid.detected for TTF-1 and CK7 et al expression by immunohistochemical MaxVision method.Results In 10 cases of lung cancer,7 cases pulmonary adenocarcinoma and 1 case small cell carcinoma expression TTF-1 and CK7,2 cases squamous cell carcinoma was unexpressed.Conclusions TTF-1 and CK7 immunostaining shows a high specificity and sensitivity for primary pulmonary adenocarcinoma,integrated pleural fluid Cell Blocks histologically suggesting that TTF-1 and CK7 could be a reliable marker for differential diagnosis of primary denocarcinoma.
